New Colorado Affirmation of Legal Status Form

The Colorado Division of Labor published recently a new version of the Affirmation of Legal Status Form (i.e. the Colorado Affirmation Form). We welcome this initiative as it also includes instructions related to how to fill out the Form, retention of documents etc. This new Form (9/06/12 version) must be used for all Colorado employees hired between October 1, 2012 and October 1, 2014.
